Open Enrollment
Help Finding Health Coverage
What to Expect
When you meet with our enrollment staff, they will start by learning a little about you and your household, including who needs coverage, your income, and any insurance you currently have. They will explain the coverage options available to you in plain language, help you check whether you qualify for lower costs or financial help, and walk you through the application step by step. If you already have a plan, we can help you review it and renew it before the deadline.
To make your visit go smoothly, please bring a photo ID for each person applying, Social Security numbers or document numbers for eligible household members, and proof of income such as recent pay stubs, tax forms, or a letter from your employer. It also helps to bring information about any health coverage you have now and the dates of birth for everyone in your household. If you are missing something, do not worry. Come in anyway and we will help you figure out what to do next.
Lifespan Is Here to Help You Get Covered
Enrollment help is available at [all Lifespan locations / specific locations] and is [free of charge, confirm]. Our staff can answer questions about co-pays, deductibles, and what your plan covers, and can connect you with other resources if you need more support. If you have had a major life change, such as losing coverage, moving, getting married, or having a baby, you may still be able to enroll outside of Open Enrollment, so please call us to find out.
Not having insurance should never stop you from getting the care you need. Lifespan Health provides services to everyone, and our Sliding Scale program may lower the cost of your visit based on your income and family size. You can learn more on our Billing Information page. To schedule an enrollment appointment, call us at [enrollment phone number] or stop by any Lifespan location during business hours.
